Step Child/Children

In order to add a step child/children to your benefit plan, please complete a Beneficiary and Census card in its entirety and send it directly to the Fund office (Contact Us).

Please also include a copy of the following documents:

• Step child/children’s Birth Certificate(s) (which should list both the Mother and Father)
• Notarized copy of your most recent tax return to show that you claim the dependent(s) as a dependent
• Notarized letter signed by you and your spouse stating the dependent(s) live(s) with you and that you provide 50% or more support and that there are no outstanding court orders that require the natural father/mother to maintain health coverage
• Marriage Certificate
